Popis: Primary hepatic leiomyosarcoma is a very rare disease, with only few cases reported in the literature. Here in, we present a case of primary hepatic leiomyosarcoma in woman age 56, revealed by abdominal pain and hepatomegaly. Computed tomography and magnetic resonance imaging (MRI) demonstrated a large mass in the right hepatic lobe associated with lung metastasis. Fine needle hepatic biopsy showed a leiomyosarcoma. Right hepatectomy was performed followed by six cycles of systemic chemotherapy (doxorubicin + ifosfamide). This combined treatment may be the optimum management of this disease. This patient experienced eighteen months of survival despite a very bulky tumor.
