In this chapter, we have attempted to answer a key question: Can a change in the organization of diabetes centers and territorial provision reduce the risk of chronic complications in patients with type 2 diabetes (T2D)? Epidemiological data show that the number of patients with T2D is rapidly increasing. This has led to the observation that current models for treating and preventing complications in such a huge number of patients are inadequate. This chapter analyzes the main models described in the literature and the role of primary care physicians, diabetologists, and diabetes centers. Finally, it deals with integrated management between primary care physicians and diabetologists, taking into account telemedicine projects currently underway. Although firm conclusions could not be drawn, in this chapter possible developments can be seen for medicine, which by its very nature is continuously evolving in both technological and cultural terms. |
