Incarcerated umbilical and ventral hernias require emergent surgical intervention in order to prevent life-threatening complications including strangulation and bowel perforation. Upon evaluation, a detailed history and physical exam should be performed with a focus on previous abdominal operations and history of ventral hernia repairs and to assess for signs of bowel obstruction, sepsis, or peritonitis. Both laparoscopic and open approaches may be used in the acute care setting. The choice of operative technique and the decision to use mesh and the type of mesh to employ are based on several factors including the presence or absence of a contaminated field, surgeon experience and preference, and patient factors. Timely recognition, early intervention, and informed intraoperative decision-making are critical to ensure optimal patient outcomes. |
