Popis: Ameloblastoma is an odontogenic tumor and, although benign, it can behave in an aggressive way and cause great deformities. Reconstruction of mandibular defects after resection is still a challenge to maxillofacial surgeons. A 24-year-old patient presented with a history of slow and painless volumetric enlargement in her posterior right mandibular region and tomographic findings of expanded and partially perforated cortical bone filled by multiple cystic areas. The clinical suspicion of ameloblastoma was confirmed by anatomopathologic examination of the specimen, obtained by incisional biopsy. Decompression was performed, evolving without success and with progressive growth of the lesion. Treatment was determined by right mandibular resection followed by perimeter maintenance with a customized total mandibular joint prosthesis at the same surgical time. Currently 6 months postoperative, the patient is in clinical and imaging periodic follow-up. The results achieved are satisfactory, and the patient is reinserted into her routine activities.
