Orthotopic liver transplantation (OLT) has been associated with major blood loss and the need for blood product transfusions. During the last two decades, improved surgical and anesthetic management and a better understanding of coagulation defects have reduced intraoperative blood loss and blood product transfusions. This chapter provides a review of the recent evolution in liver transplantation. We discuss the current state of the literature for transfusion guidelines and blood product management in this particular patient subset and summarise a variety of strategies (pharmacologic and other) to reduce the need for transfusion. Our goal is to review the latest published evidence on these topics and highlight areas that need further investigation to optimize the clinical care of these patients. |
