Efective care for diabetic and hypertensive patients has a substantial impact on the national health system finances. The intervention costs of chronic patients are very high and due to the untimely attention they are causing high mortality and morbidity rates representing a social problem to the health system. To minimize this impact, this article presents a description of devices technological on IoT that seeks to monitor at home physiological variables to determine the health status of the patient remotely. This research is being developed by the DAVINCI of Universidad Nacional Abierta y a Distancia de Colombia (UNAD) and the University of Jaén from Spain (UJAEN), in collaboration with the Colombian Red Cross Organization Section Antioquia and with REMIND research project from European program H2020. The article is organized in three parts: 1) Chronic diseases telecare delivery models used in Colombia, Spain and Costa Rica, 2) Integration of IoT devices applied to a chronic care delivery model, and 3) Sensors for homecare monitoring. This document presents the identification of devices, looking to validate the technological model and to evaluate benefits and savings using this monitoring health service model for diabetes and hypertension.  |
