Popis: Introduction: Complex abdominal deformities or surgical sequelae present hard to solve issues, sometimes challenging the surgeon’s technique and expertise. The difficulties in the handling of those patients are caused by the harsh conditions of the abdominal cutaneous tegument, where we can find multiple surgical scars, adherences, retractions, and cutaneous projections, coming from traumatic or iatrogenic fat tissue injuries. Case by case and methods: The authors show three complicated and challenging cases with defects of the abdominal wall to be worked on aiming not only to reconstruct the previous abdominal wall but also the entire outline of the body. Discussion: Beyond following tendencies, abdominal aesthetic surgery needs a more global focus in relation to the corporal outline. The combined approach of all the thoracic-abdominal areas and the right treatment of those modifications could allow a much better harmony of the body’s contour. Conclusion: The cases shown require that the surgeon modify the approach standards, using unusual conduct, therefore respecting the established principles to achieve more refined and aesthetically surprising results.
