A CASE OF ACUTE COLONIC PSEUDO-OBSTRUCTION FOLLOWING PARTIAL DESCENDING COLECTOMY FOR COLONIC CANCER

Popis: Acute colonic pseudo-obstruction (ACP) is characterized by prominent gaseous colonic dilatation without organic lesions. This syndrome should be treated carefully because it resembles paralytic ileus but is far different from the latter in etiology and clinical course. We report a case of ACP in a 61-year-old man who had past history of left nephrectomy 20 years ago and underwent partial descending colectomy for colonic cancer. His early postoperative course was uneventful, but mild abdominal distention and gaseous colonic dilatation developed about 10 days after the operation. When nasogastric suction therapy failed to dissolve the progression of the abdominal distention and colonic gaseous dilatation, cecostomy was performed on day 22. Prompt improvement was observed after cecostomy, and a contrast study of the entire colon via the cecostomy revealed no evidence of mechanical obstruction, including adhesions, colonic torsion or anastomotic failure.
