Popis: | We report on using the tibial tubercle osteotomy for exposure of TKA revision surgery, after high tibial osteotomy and contractured knee in which the usual approach is not as Scutable due to eversion of the patella caused by soft tissue contracture or adhesions and shortened extensor mechanism. We introduce our method and report the outcome. We used this method for 14 cases (16 knees, primary TKA: 2 cases 2 knees, revision: 6 cases 8 knees, post high tibial osteotomy: 5 cases 5 knees, contracutured knee: one case one knee). We approaded the knee joint with tibial tubercle osteotomy leaving the lateral soft tissue and periostium intact. When we close the wound we fixed the osteotomised tibial rubercle with 2 screws and after surgery rehabilitation was done as usual except for 3 weeks when a knee brace dressing was used only during walking. This method had no fixation failures of the tibial tubercle or nonunion and no complications. We consider this method aclueves complete exposure with ease and is useful for performing TKA with eversion of the patella. |
