Intestinal anastomosis can be performed in many ways. To date, there is no standardized method that all surgeons agree upon. With the advent of stapling devices, most surgeons agree that intestinal anastomoses have been greatly simplified by using these instruments. However, there are multiple stapling devices and multiple ways of completing an anastomosis without one way showing superiority over another. This is true in traditional open surgery and laparoscopic surgery. In general, laparoscopic intestinal anastomosis is performed in either an extracorporeal (including hand assist) or intracorporeal manner. There are no randomized controlled trials demonstrating the best technique. Over the years, I have migrated to an intracorporeal anastomosis for ileocolic, right colon, left colic, and occasional high ileosigmoid anastomoses. This chapter will detail the advantages of intracorporeal vs extracorporeal, show the technique in detail, and discuss the skillset required to complete the anastomosis. |
