Brain metastases as the initial site of failure in neoadjuvant chemotherapy patients (pts) with operable non-small cell lung cancer (NSCLC): Implications for prophylactic cranial irradiation (PCI)

Popis: 7327 Background: Between 25%–30% of all pts with NSCLC develop BM at some stage of their life, and this percentage is likely to increase with lengthening survival of pts with early stage of disease at presentation. The aim of the present study was to evaluate the pattern of recurrence after curative resection and identified potential pts for PCI. Methods: We reviewed the outcomes of 30 pts operated on for pathologic stage IB to IIIA NSCLC between Jan 2000 to Aug 2002. Pts received platinum doublet neoadjuvant chemotherapy(CT)in the context of phase II study. If considered operable, had a mediastinoscopy. Pts with negative mediastinal lymph nodes were operated with curative intent. Pts with involved mediastinum nodes were treated with xRT. The histologic type was squamous carcinoma (sqc) in 12, nonsquamous carcinoma (nsqc) (13 adenocarcinoma and 1 large cell carcinoma) and 4 nsclc. Mean follow up was 40 months.
