Popis: | The three cases represent the range of GI bleeding that may be observed in pediatric patients with status asthmaticus. Possible etiologies include drug use such as theophylline, corticosteroids, and beta sympathomimetics as well as stress ulceration. Since the GI bleeding occurred within 12 hours of initiating corticosteroids, it is unlikely that they alone were responsible. Stress-related peptic ulcers occur in 6 to 100% of critical care patients. Ischemia seems to be the major cause of stress ulceration. Given the time course, it seems most unlikely that stress through tissue hypoxia and ischemia played a major role in the etiology of the bleeding in the last 2 cases. Currently, the prevalence of GI bleeding in children with status asthmaticus is not known, nor have the risk factors been delineated. Although all three patients reported in this series were black, they do not indicate a propensity for GI bleeding in this ethnic group; rather, they may reflect the referral pattern to a tertiary care center. |
