Popis: | Thyroid disease is the most prevalent endocrine disorder (next to hyperglycemia) in pregnancy worldwide. Hypothyroidism is more common and affects women in both iodine sufficient and deficient areas. In areas of iodine sufficiency, autoimmune thyroid disease accounts for the majority of overt hypothyroidism and thyroid antibodies found to be detectable in up to 60% of pregnant women with an elevated TSH. Subclinical hypothyroidism remains a subject of controversy, as studies show variable outcomes. Hyperthyroidism due to Grave’s disease affects about 0.2%–0.4% of pregnant women, followed by gestational hyperthyroidism and toxic adenomas. Postpartum thyroid disorders are important to recognize, given their prevalence and impact on maternal and neonatal health. This chapter reviews in depth the diagnosis and management of the continuum of thyroid disorders, including issues arising in the preconception and postpartum periods, discusses the available evidence and clinical guidelines of thyroid disorders in pregnancy. |
