Popis: Chronic renal insufficiency (CRI) in children is often accompanied by malnutrition; the extent to which a child's nutritional status is compromised is determined largely by the severity of his or her renal failure. Many of the clinical manifestations of the uremic syndrome, including susceptibility to infection and impaired wound healing, are magnified in the concurrent presence of malnutrition. Because both malnutrition and CRI retard growth and development, the nutritional status of affected children must be assessed to determine appropriate nutritional support. The present review focuses on the methodology of such an assessment. A dietary history and physical examination can be employed in most clinical situations. Generally, anthropometric and blood and urinary biochemical parameters are used to assess nutritional deficiencies and evaluate response to treatment. Careful serial measurements can provide accurate indices of the efficacy of therapy. Serial measurement of the child with CRI both clinically and systemically provide the framework for the continual monitoring of the patient's growth, development, and nutritional status.
