Aspiration is one of the major complications encountered after large oral and oropharyngeal ablative procedures such as resections of lingual, tonsillar, and pharyngeal wall carcinomas. During 1991 through 1995, seven patients with T3 and T4 oral and oropharyngeal carcinoma underwent large, crippling, oropharyngeal resections along with laryngeal inlet closure by epiglotto-aryepiglottopexy (EAP) to prevent potentially associated, life-threatening bronchial aspiration. This laryngoplastic technique permits laryngeal retention, prevention of aspiration, and preservation of phonation. The operative techniques are detailed, and the literature dealing with the subject is reviewed. |
