The tissue diffusivity of pelvic cancers is usually lower than most benign processes and is reflected by a lower ADC value. When performing DW-MRI in the pelvis, avoidance of air in the bowel or rectum, together with the optimization of imaging pulse sequences are important to achieve optimal image qualit y. In the pelv is, DW-MRI using b-values of 800–1,000 s/mm2 with the application of parallel imaging can be recommended to achieve sufficient suppression of the background signal. DW-MRI is showing added value for the detection and characterization of cancers, particularly in the prostate and in gynaecological malignancies. DW-MRI may also play a role in lymph node characterization, disease prognostication and tumour response assessment in a variety of pelvic malignancies such as prostate and rectal cancer. The application of pelvic DW-MRI continues to evolve and further studies will help to establish its role in clinical practice. |
