In the United States, food insecurity is a determinant of health that impacts 30% of single-mother households. Within Cumberland County, NC, 40.4% of households with children are single-parent households, and as 80% of single parents nationwide are women, this suggests that the economic burden faced by Cumberland County single parent households most heavily impacts single mothers. To address this, an Accountable Care Community (ACC) will be formed, convening community partners who will implement two interventions: the provision of medically tailored home-delivered meals to food insecure mothers, as well as expansion of Supplemental Nutrition Assistance Program benefits and education in the county. The ACC aims to reduce food insecurity and simultaneously improve chronic health indicators and healthy eating habits within the priority population. The ACC partners will include the Public Health Department, the Cape Fear Valley Hospital, the County Meals on Wheels, community advocates, and the Food is Medicine Coalition. |
