Spirometric and clinical profiles in asthma copd overlap (ACO)

Popis: Introduction: ACO is an heterogeneous group with different clinical phenotypes. The aim of this study is to determine the relationship between clinical profile and spirometry. Method: 53 patients were included between 2016-2019 in a retrospective study. The diagnosis of ACO was made by GINA 2018 criteria. Clinical profile and spirometry were collected. Statistical analyzes were performed with SPSS 25.0 software. Results: Fifty-three patients present the ACO criteria of GINA 2018. The mean age is 62 years. Sex-ratio M/F is 7.8. Smoking rate is 90% with mean pack-year at 44. An allergic history is founded in 18.9% of cases. Paroxysmal dyspnea is more frequent than effort dyspnea (79% vs 52%). 49.1% of patient present an acute symptom only, 5.7% present only chronic dyspnea and 45.3% with mixed symptoms. The mean FCV max is 2980ml. The mean FEV is 1782 ml (58% of predict). The reversibility is significant in 35%. The mean FEV reversibility is 217 ml (16% of predict). The variability in time is present in 39.6% with mean variability = 181 ml (7% of predict). This variability is correlated with late onset of symptoms (p=0.011). Association LABA/LAMA is used on 71.2% of patients: 35% are well controlled, 47% partially and 17% not controlled. The mean FEV1 and mean FEV1 variability in well controlled group and not controlled group is respectively 2125 ml and 240 ml vs 1563ml and 159ml with respectively (p=0.026) (p=0.05). Conclusion: The correlation between clinical profile and spirometry is certain. The renewed interest seems obvious.
