Popis: | Decisions to treat in all disease states or not to treat imposes a total approach for rationale judgements in patient care. Ethical issues to dialyze an old person, to initiate chemotherapy in a nonagenarian, or to withdraw treatment all can impact on the physiologic changes with age, the underlying pathophysiology of end stage renal disease (ESRD) and either enhance pharmacodynamic events or cause them to cease by withdrawing treatment. The following discussion is an attempt to document the critical interplay between nephrologist and geriatrician, patient and next of kin in clinical decision-making involving the ethic of dialysis in the elderly. |
