In late pregnancy sphygmomanometry of the brachial artery does not give a reliable estimate of the simultaneous pressure in the arteries which are distal to compression exerted on the aorta by the gravid uterus. 3–5 Distal arterial depression is of special importance for the obstetrician because it reduces uteroplacental blood flow, a determinant factor of maternofetal interchanges. Femoral brachial regression diagrams permit an estimation of the unknown pressure in the distal region from the sphygmomanometric value measured on the arm. The systolic pressure is approximately equal to the sphygmomanometric value divided by .07, less 58 mm. Hg. The diastolic pressure is always lower than the sphygmomanometric value, by 20 mm. Hg during hypotension, by 12 mm. Hg during normotension, and by 8 mm. Hg during hypertension. |
