Aneurisma de carótida interna y estenosis carotídea preoclusiva bilateral

Popis: Summary Introduction. Internal carotid artery aneurysm is a very rare disease specially extracraneal with carotid stenosis. Case report. A 75 years-old man, with a history of severe coronary heart disease, had a hemisphere transient ischemic attack (TIA) followed by a progressive cognitive decline. Dupplex ultrasound examination showed bilateral preocclusive carotid stenosis. Anticoagulant treatment was started because of his high cardiologic risk, but the patient experienced a new right hemisphere TIA. Thus, surgical treatment of the symptomatic carotid stenosis was decided. Carotid angiogram confirmed preocclusive carotid lesions and identified a right internal carotid loop. During the intervention a 20 mm of diameter aneurysm was found, 3 cm beyond the internal carotid artery origin, according with this finding the initial purpose changed: carotid endarterectomy with simple suture and aneurysm resection with termino-terminal anastomosis of internal carotid artery were performed. Surgery stopped cerebral ischemic attacks and an important improvement in cognitive decline was achieved. Conclusion.
