Popis: | Congenital segmental megaureter with sparing of the proximal and distal portions of the ureter is a rare condition, with only 1 previously reported case to our knowledge.' We report a case of bilateral segmental megaureter posing diagnostic and therapeutic problems. CASE REPORT A 12-yearsld boy was born with multiple congenital anomalies, including high anorectal agenesis, vertebral, sacral and urological anomalies, and malformations of the right upper extremity (VACTERL association). The nonurological malformations were treated surgically. The urological malformations consisted of marked dilatation of the right ureter with comparatively mild hydronephrosis, and a small left hydronephrotic kidney with poor excretion of contrast material, which contributed less than 10% to overall renal function. Voiding cystourethrography was refused by the parents. Consultant opinion was to follow the patient conservatively, since surgical correction of these urological malformations might carry too high a risk. The rare pyelonephritic attacks and frequent asymptomatic bacteriuria were medically treated. The child was referred to our hospital 11 years later for treatment of renal pathology. Creatinine was 135 pmo1.A. (normal less than 100) and endogenous creatinine clearance was 75.6 ml. per minute per 1.73 m? (normal greater than 80).
