Popis: | Left ventricular assist devices (LVADs) have successfully been used as both bridge (to transplantation or decision) and destination therapy for patients with dilated cardiomyopathy. However, there is still very limited experience with using LVAD therapy in patients with advanced heart failure (HF) and nondilated left ventricle (LV). This chapter reviews and discusses recent applications of LVAD in the treatment of restrictive cardiomyopathy and cardiac amyloidosis. The critical points of LVAD implantation and management in patients with small LV cavity are highlighted. In addition, there are reports of successful LVAD therapy in patients with a failing systemic ventricle in Fontan circuits. This success suggests that with further advances in LVAD technology, such as miniaturization of the pump’s inflow cannula and improved hemocompatibility, LVAD can become a treatment modality for advanced HF in patients with other nondilated cardiomyopathies, most importantly, heart failure with preserved ejection fraction. |
