Aortic disease is well known. The most common lesions involving the aorta include the aortic aneurysms and the atherosclerotic obstructive lesions. Each of these may also be involved in additional complications of the abdominal aorta—namely, aortic fistulas. Primary and secondary aortic fistula may develop in the patient with an aortic aneurysm or postsurgical therapy for an aneurysm. Similarly, after open surgical grafting of an obstructive atherosclerotic aortic lesion, a patient may develop a secondary aortic fistula. Aortogastric or aortoenteric (duodenum, jejunum, ileum, colonic, appendiceal) and even biliary fistula may result from primary or secondary causes such as infection, surgery, or trauma. Initial treatment, after appropriate diagnostic evaluation, will include endografting in most patients. After stabilization, many patients will then require an open graft placement in order to heal the infected aorta and close the defect. Sepsis and blood loss are then therapeutically under control. |
