In a recent series of patients with intractable epilepsy studied at Queen Square the advent of high-quality MR volume imaging enabled the pick-up rate for many abnormalities to be raised significantly (Free et al., 1992). Of 128 patients studied, 110 were CT negative and 29 had a previously negative MR scan. In 92 of the CT-negative cases an abnormality was identified on volume MR, including hippocampal volume loss, foreign tissue lesions, macrogyrias and band heterotopias. However, this still leaves 18 cases in which no abnormality was shown. In some cases of cortical dysplasia clinical opinion was equivocal, and in others the extent of the abnormality was unclear. The use of high-quality volume imaging gives us the opportunity of extending our understanding of these abnormalities with the techniques of digital image processing. |
