Surgical correction of mandibular prognathism in preparation for dentures

Popis: Many of the past difficulties associated with the surgical correction of the prognathic mandible can be avoided. The flattened appearance of the chin, the derangement in the structures in the floor of the mouth, and the possibility of delayed union or nonunion of the parts caused by the technique of ostectomy in the body of the mandible are avoided. Also, with the technique of horizontal osteotomy of the rami there is the possibility of an increased anterior vertical dimension. The possibility of hemorrhage and the tragic sequelae of the proximal fragment being displaced medially can be avoided by using the open approach to the vertical subcondylotomy. The postoperative jaw position in these future denture patients should be determined by the prosthodontist. He can mount the casts arbitrarily in a postoperative position, regardless of the remaining teeth, taking into consideration the ridge relationship for the dentures to be made sometime in the future.
