Central nervous system ICNS) involvement is a frequent manifestation of systemic lupus erythematosus ISLE), and it has been thought for some time that it carries a poor prognosis. However, some recent reports suggest that CNS involvement may be a serious, but not fatal complication. 1 There is a consensus that corticosteroids are the mainstay of therapy. But some patients do not improve with corticosteroids, while others show long-term sequelae of acute CNS manifestations despite high-dose corticosteroid or immunosuppressive therapy. We have 'used plasma exchange IPEI when such doses of corticosteroids failed to control severe acute CNS involvement, and we report seven severe neurologic episodes in six patients in which the use of PE was determined by CNS involvement, independent of other visceral manifestations. |
