Paroxysmal atrial fibrillation in pregnancy

Popis: Case report The medical obstetric team were asked to see a 27-year-old lady in the antenatal clinic regarding her recent palpitations. She was 32 weeks into her second pregnancy, having previously suffered a first trimester miscarriage. Since the age of 12 she had experienced occasional palpitations associated with some difficulty in breathing. Each attack lasted approximately 5 min resolving spontaneously. They occurred very infrequently (once every l-2 years). These palpitations had never been investigated and had not concerned her. However, since 16 weeks gestation the attacks had become more frequent and more severe. She noted four symptomatic episodes lasting up to half an hour and on the most recent occasion she felt close to fainting. There appeared to be no precipitating factors in terms of alcohol, excess caffeine or stress. She was a non-smoker and there was no history of drug abuse. She had no other significant medical history. An ECG had recently been performed at another hospital during one of the attacks and this confirmed the presence of atria1 fibrillation with a ventricular rate of 150 beats per minute (b.p.m.) There were no other ECG abnormalities. Examination at the antenatal clinic revealed no signs of thyrotoxicosis or heart failure. She was in sinus rhythm with a heart rate of 80 b.p.m. and blood pressure 120/75 mmHg. Clinically the heart was not enlarged and there were no heart murmurs detected. Her chest was clear on auscultation. Fetal growth was satisfactory. Thyroid function tests
