A rare case of malignant lymphoma arissing in the residual stomach 8 years after distal gastrectomy for a gastric cancer is reported. Thirty-five such cases seen in the Japanese literature are also reviewed and discussed. A 72-year-old man was admitted to the hospital because of epigastric disconfort. There was a previous history of undergoing distal gastrectomy (Billroth I) for an early gastric cancer 8 years before. A nodular protruding lesion was detected by upper G.I. series and endoscopic biopsy of the stomach revealed a histological diagnosis of malignant lymphoma of the stomach. Total excision of the residual stomach was performed. The pathological diagnosis was diffuse medium-sized cell type of malignant lymphoma in LSG's classification, with the invasion depth of t3 (se) and n0. Postoperatively, the patient received two courses of chemotherapy (VEPA). The patient is alive without any signs of recurrence of malignant lymphoma, as of one year after the operation. In a review of the literature, the majority of malignant lymphomas in the residual stomach were in far advanced stage, but the prognoses of resected cases were comparatively good. It is thought that aggressive resection with postoperative chemotherapy is recommended for treating malignant lymphoma in the residual stomach. |
