Solitary splenic abscess is relatively rate. A 73-year-old man who was treated for cerebral infarction and atrial fibrilation at elsewhere had slightly degree of fever and abdominal pain after he left that hospital. The patient developed high fever and was admitted to the hospital for examination. He was associated with senile dementia. Abdominal CT revealed a 11×10×13cm solitary splenic abscess. It was supposed that the operation for this case had a high risk, so chemotherapy with various antibiotics was started. The tumor, however, did not remarkablely diminish, abdominal pain lasted, and no meaning to continue the conservative therapy could not be find. Splenectomy was performed. The specimen was consisted of a 6×4×11cm solitary abscess in the splenic parenchyma, scarformation around the abscess and multiple thrombi in the splenic artery. These findings suggested that the splenic abscess resulted from the splenic infarction. |
