'Still ticking successfully'—Bentall operation with handmade conduit using TTK Chitra valve—midterm analysis

Popis: Modified Bentall procedure is the gold standard procedure for several aortic root diseases. The aim is to study the short- and midterm results of modified Bentall procedure using intraoperative custom-made composite graft with TTK 61Health Care Limited, Thumba,Trivandrum, Kerala, India (TTK Chitra valve). Between January 2006 and December 2013, 100 patients underwent aortic root replacement using rigid tilting disk TTK Chitra heart valve as the composite graft valve. This study includes patients between the age group 19–62 years who had Chitra valve in the composite graft. A single surgeon was involved during the entire period. Data were collected retrospectively from operative records, hospital charts, and clinical follow-up notes. Early and late mortality and morbidity were assessed. Left ventricular function parameters were compared preoperatively and during follow-up for a mean duration of 32.52 + 24.34 months (1−8 years). Valve functions were also assessed. Seventy-two patients (72%) were male, and 28 patients (28%) were female with a mean age of 42.07 + 10.90 years. Twenty-nine patients (29%) had Marfan syndrome. Six patients (6%) received a concomitant cardiac procedure. The mean aortic cross-clamp time was 103.01 + 19.72 min, and the mean cardiopulmonary bypass time was 136.11+ 24.84 min. The early operative mortality was 2% (two patients). The late mortality was 3% (n = 3). Significant left ventricle (LV) remodeling was noted in 85% (n = 85) of the patients. Midterm survival is good. Bentall procedure can be done safely with intraoperatively prepared composite graft with any existing valve. The Bentall procedure using intraoperatively prepared composite graft using TTK Chitra valve is safely used in many countries, and it offers excellent midterm results. The results are comparable to other series of composite graft study making it an effective, accepted, safe, and cost-effective root replacement prosthesis.
