Popis: | Patients with end-stage liver disease have complex rebalanced haemostasis characterised by reduced pro- and anticoagulant factors and reduced reserve to compensate homeostasis. During liver transplantation, portal hypertension and surgery can contribute to sudden and massive bleeding, as well as thromboembolic events. Perioperative monitoring of haemostasis is paramount for rational blood and blood products transfusion and haemostatic maintenance. In this chapter, we will discuss perioperative haemostasis monitoring, which includes clinical observation of the operating field, traditional coagulation laboratory test and point-of-care (POC) haemostasis monitoring. Additionally, we will present guidance, ‘how to use POC laboratory tests to support coagulation and transfusion management during liver transplantation’. Finally, we will present current evidence showing how POC testing can improve outcomes and reduce the costs of liver transplantation. |
