Paravalvular abscess is a serious complication of prosthetic valve endocarditis with various clinical presentations. In this report, we present a case of paravalvular septate abscess with demonstrative echocardiography images and complete atrioventricular block (a.v.) one month after aortic valve replacement. Transthoracic echocardiography was performed when complete av block was detected in the electrocardiography of the patient who presented with the complaint of fatigue and shortness of breath. Then transesophageal echocardiography was performed due to suspicious paravalvular abscess around the mechanical aortic valve in transthoracic echocardiography. In transesophageal echocardiography, paravalvular septate abscess image was detected. In patients with prosthetic valves, newly developing av block should be a warning for paravalvular abscess.
