Pharmacology of Bismuth-Containing Medicines Used to Treat Helicobacter pylori Infections

Popis: Although bismuth-containing compounds have been used for centuries, it is only in recent years that a systematic investigation into their chnical efficacy and mechanisms of action has been carried out. This was stimulated by scientific advances in the fields of diarrhoea and peptic ulcer. Both the recognition of the importance of Helicobacter pylori in the aetiology of peptic ulcer disease [3, 9, 10, 16, 20] and the finding that bismuth-containing medicines (BCMs) have a profound effect on the organism [17, 23, 32] have been a great spur to research into the actions of BCMs. The best known BCMs used to treat H. pylori infections are colloidal bismuth subcitrate (CBS: DE-NOL, Telen) and bismuth subsalicylate (BSS; Pepto-Bismol, Iatrox), and to a lesser extent bismuth subnitrate (BSN; Roter). A comparison of the actions of these compounds on H. pylori is hampered by the shortage of published information on BSS and BSN. In this review I discuss briefly the chief physical chemical properties of the bismuth compounds, followed by a discussion of their anti-microbial action and their effects on the postulated pathogenic mechanisms of H. pylori.
