According to the currently accepted model of basal ganglia circuitry, cortical information is transmitted to the output stations of the basal ganglia through two main pathways, referred to as the direct and indirect pathways.1 The direct pathway consists of a GABAergic projection from the striatum to the internal globus pallidus (GPi) and the substantia nigra pars reticulata (SNpr). The indirect pathway connects the same structures via (i) a GABAergic projection from the striatum to the external globus pallidus (GPe), (ii) a GABAergic projection from the GPe to the subthalamic nucleus (STN), and (iii) a glutamatergic projection from the STN to the GPi. This model has been a great help in understanding the functioning of the basal ganglia in normal, hypoactive and hyperactive conditions. Recently however, a number of limitations have appeared in the model and it has had to be revised.2,3 4 |
