Unclassified Sarcoma of the Skull Base: Review of 11 Cases

Popis: Introduction: Even with maximal diagnostic effort using current histological techniques. a fraction of sarcomas cannot be classified according to their tissue of origin. We studied the prognosis of these tumors with skull base involvement. Methods: A retrospective review of 11 patients (4 male, 7 female; mean age 46 y, range 6–81 y) from two institutions with the diagnosis of unclassified sarcoma involving the skull base from 1993 to 2003 was undertaken. Results: All patients underwent open surgery, 10 for cure (7 were salvaged), one for biopsy. Gross total resections were achieved in 9 (microscopically negative margins in 6) mostly via piecemeal removal (n = 6). Six patients underwent adjuvant radiation and/or chemotherapy. Three declined adjuvant therapies and 2 had no adjuvant treatment offered. Overall survival was 71.4 months (range 2–116 mo). Although all sarcomas remained unclassified, 5 were histologically low, 3 intermediate, and 3 high grade. The overall survival averaged 78.3 months (range 30–116 mo) after curative treatment. At last follow-up, 7 were alive (5 without evidence of disease). Five-year progression-free survival was 64%. One late relapse was recorded 9 years after index treatment. Average overall survival of patients with curative treatment divided by grade was 35.7 months for high-, 92 months for intermediate-, and 87.2 months for low-grade tumors. Conclusion: The majority of unclassified sarcomas were of low histological grade. With multimodality management, the majority (n = 7) had no progression at 5 years. Overall survival for high-grade lesions was worse. Due to the possibility of delayed relapse, continued follow-up is critical.
