Popis: Nonalcoholic fatty liver disease is the most common inflammatory liver disease and is associated with a substantial risk of developing clinically significant, inflammation-driven, liver fibrosis. As a syndrome, it reflects the hepatic manifestation of metabolic syndrome for the majority of patients and as such is a complex result of interacting genetic, metabolic, and environmental risk factors. While clinically outcome for patients is complex because of the interwoven systemic metabolic/cardiovascular risks, in the liver itself, steatosis, steatohepatitis, progressive liver fibrosis, and hepatocellular carcinoma evolution are contributed to clearly by interacting innate and adaptive immune responses. These responses are modulated by cellular responses to inflammation and contributed to externally by microbiome alterations. Understanding the architecture of the immune response to hepatic fat deposition, in the setting of cellular insulin resistance, is facilitating new approaches beyond metabolic and anti-fibrotic themes for future targeted interventions.
