A 74-year-old male was admitted to the hospital because of left upper quadrant pain. A huge tumor with tenderness was palpated at the same region. Since abdominal echogram, CT and scintigraphy showed a splenic tumor and cholecystolithiasis, splenectomy with regional lymph node dissection, cholecystectomy and liver biopsy were done without any complication. The excised spleen, weighing 1, 090g with the size of 19×13×10cm, revealed the atypical lymphocytes forming a follicular pattern by microscopic examination. The diagnosis of follicular lymphoma of the spleen, mixed type was made according to the LSG classification. There were some involvements into the nodes but not in the liver. Only 5 cases of the splenic follicular lymphoma have been reported in Japan since the LSG classification was adopted. There has been no recurrence, as of 8 months after the operation followed by three courses of CHOP therapy. |
