We encountered a case of Crohn's disease which first manifested with pain the lower abdominal region and was diagnosed after the occurrence of right hydronephrosis. The patient was a 25-year-old man. Pain in the right lower abdominal region, fever and leucocytosis had recurred since August 1993, and appendectomy was performed under a diagnosis of appenditis. Slight fever and pain in the inguinal region, however, persisted even after the operation. Mild right hydronephrosis was noted on CT-scan on September 13. Since the pain became more intense and leucocytosis was noted on September 22, DIP was performed. Marked hydronephrosis and hydroureter were observed. Intensified retroperitoneal inflammation and ureteral stenosis were diagnosed, and surgery was performed. The ileum was found to be firmly adhered to the retroperitoneal abscess, and inflammation was noted to have reached the ureter. The abscess was released and about 30cm of the ileum was resected. Crohn's disease was diagnosed on the basis of macroscopic and pathological findings of the specimen. Hydronephrosis remitted promptly after the operation. |
