Lung adenocarcinoma mimicking interstitial lung disease showed good response to navelbine monotherapy

Popis: Introduction Clinical presentations and radiologic findings of lung adenocarcinoma often resemble benign pulmonary diseases such as community-acquired pneumonia and interstitial lung disease. We report a lung adenocarcinoma with widespread ground-glass opacities(GGO) mimicking interstitial lung disease, also it had shown remarkable response as complete remission to navelbine. Case Presentation A 66-year-old male patient admitted our hospital for cough, sputum and dyspnea aggravated 2 days ago. He also had suffered from chronic cough for one year. Chest CT revealed widespread GGO with patchy consolidations(Fig1A). After 2 weeks, we decided to perform open lung biopsy due to absence of improvement despite proper antibiotics. It revealed as adenocarcinoma with visceral pleural invasion. Although the patients had treated with multiple chemotherapies such as platinum doublet, erlotinib, gemcitabine and docetaxel for 2 years, disease progressed slowly. Finally, we treated with navelbine weekly at fifth line for 12 months and GGO was disappeared dramatically. It resulted in complete remission and maintained for 11 months(Fig1B). Conclusions Lung adenocarcinoma sometimes mimics benign lung disease. It is prone to delay diagnosis of cancer. Open lung biopsy will be a useful tool for precise diagnosis. Also, we discreetly consider navelbine could be a treatment option for chemotherapy refractory lung adenocarcinoma.
