Popis: | This study describes how data from a mental health service system costeffectiveness study was used to develop and test a model for estimating savings in mental health inpatient care reimbursed by Medicare. Simulation resulted in estimated actual savings per admission which vary according to covered length of stay and per diem unit costs, but average at 20% of the Medicare reimbursement. When the model is applied to DRG430 (psychoses) admissions, the savings increased to about thirty percent. The authors caution that costcontainment limited to fiscal incentives may have negative consequences for patient care. Program restructuring designed to better balance the service system through levels of care may be more clinically sound, and less costly, than Medicare's current prospective system. |
