Popis: Publisher Summary The combination chemotherapy and high-dose cisplatin and bleomycin infusion is highly effective as induction therapy in previously untreated squamous cell head and neck cancer. It is well tolerated with minimal and transient toxicity when cisplatin is administered with mannitol diuresis and hydration. This induction chemotherapy increases the probability of local control in patients presenting with advanced disease. Patients showing a major response to this combination experience a better overall and disease-free survival. As with other malignancies, the achievement of complete remission of disease, which increases the probability of overall and disease-free survival, is important to the multimodality approach in patients with advanced disease. It is probable that such therapy may have significant beneficial effect on micrometastases at the time of initial presentation. Longer follow-up and prospectively controlled studies are necessary to further define the role of induction and maintenance chemotherapy. The National Cancer Institute in a multiinstitutional study has explored the usefulness of induction and maintenance chemotherapy in resectable advanced head and neck cancer.
