Detecting COPD exacerbations through Prevexair App

Popis: Introduction: The treatment of chronic obstructive pulmonary disease exacerbations (AECOPD) improves the outcomes; however, patients with COPD often fail to seek therapy. Most exacerbations are not reported and probably not treated despite their association with a lower life quality and increased risk of frequent hospitalizations. This pilot study was aimed to determine the frequency of AECOPD symptom-based definitions on a day-to-day basis and whether these exacerbations are reported in daily clinical routine. Material and Methods: We obtained data from a multicenter, observational study, with prospective recruitment of patients with COPD and frequent exacerbations (≥2 moderate exacerbations in the previous year and /or ≥1 severe exacerbation in the previous year). All the patients registered via smartphone app Prevexair their daily respiratory symptoms, medications as well as any medical assistance for 6 months. Exacerbations were defined by standardized criteria of the London COPD cohort. Results: 116 patients were evaluated. The mean age was 66.5 (8.1) years, 78.4% were males, 13.8% active smokers, with FEV1 44.6 ± 16.2%, a dyspnea grade ≥2 was present in 74.1% and 53.4% with ≥ 1 severe exacerbation / per year. Only 12.1% of patients did not report exacerbations versus 73.3% patients who did report them. A total of 81 exacerbations defined by symptom were not reported whereas 180 were reported, requiring hospital assistance in 19 of them. Conclusion: This study provides further evidence on the feasibility of early prediction of COPD exacerbations based on the daily patient’s self-report of symptoms through a monitoring tool. Prevexair app may also allow to track and even report different symptoms.
