The value of EBUS–TBNA in the diagnosis of lymphoma. The experience of a single centre from Romania

Popis: Endobronchial ultrasound-guided transbronchial needle aspiration (EBUS-TBNA) offers a minimally invasive way to evaluate mediastinal tumors and adenopathies. The aim of our study is to evaluate the value of EBUS-TBNA in the diagnosis of lymphoma. Method: A retrospective analysis of 442 EBUS-TBNA examinations with rapid on-site examination (ROSE) and histopathological examination, performed for diagnostic purposes between 2014 and 2018 in the Department of Bronchology of Cluj-Napoca Pneumology Clinic was conducted. The examinations were carried out under general anesthesia, using 22 G needle with a mean of 3 passes per lymph node. We present our experience with EBUS-TBNA as a tool in patients with lymphoma suspicion. Results: In our retrospective study we analyse the data of 442 EBUS-TBNA examinations with ROSE and histopathologic exam. The age of the patients varied from 21 to 80 years, with an average age of 54.36 years. From the total number of examinations 24 (5,42%) had mediastinal tumors and mediastinal adenopathies with lymphoma suspicion. Out of 24 cases we identified malignancy at ROSE in 18 cases (75%), in 4 cases (22,2%) lymphoma cells and in 14 cases tumoral cells. Histopathological exam revealed lymphoma in 22 patients (91,6% of the presumptive diagnosis of lymphoma). The sensitivity of the ROSE in lymphoma diagnosis in our study was very low (18,18%). Conclusion: EBUS-TBNA can be considered an important diagnostic tool for patients with mediastinal tumors and lymphadenopathy suspected to be lymphoma. By the introduction of this method EBUS-TBNA with ROSE in our country we can diagnose patients with lymphoma that cannot be diagnosed by conventional bronchoscopy.
