Resident duty hours have recently come to the forefront of resident education and nowhere is this more acutely felt than in the surgical disciplines. Our past was marked by duty hours that were draconian by most standards and associated with emotional, psychological, and physical damage to the trainees. Changes in this system began to occur once the impact of these long hours was seen to negatively impact patient care and outcomes; unfortunately, especially in surgery, it was felt that the education of competent, knowledgeable, and prepared surgeons was sacrificed as a result. Recently, high-quality studies of this issue that incorporate both patient outcomes, resident education, and resident well-being have been conducted to provide rational data to drive the discussion. With this, some of these restrictions in duty hours for surgical residents have been relaxed to allow residents themselves to honor their obligation to their patients and their craft while maintaining a safe working environment. In this chapter, we aim to review the past, present, and foreseeable future as it applies to surgical trainees. |
