To The Editor: Having read “Reverse Obliquity Fractures of the Intertrochanteric Region of the Femur” (2001;83:643-50), by Haidukewych et al., I was surprised by the authors’ lack of perspective on the treatment of reverse obliquity subtrochanteric fractures of the femur. I was particularly interested in their statement, “to our knowledge, there are no published reports in the literature that specifically address the results of treatment of this particular fracture pattern.” I would strongly suggest that the authors read the article “An Intramedullary Fixation Device for the Proximal Part of the Femur. Nine Years’ Experience” (1976;58:866-72), which was a follow-up analysis of eighty-four nonpathological fractures treated with the Zickel nail device. Of particular interest was a morphological classification, well illustrated on the first page of this article, noting the oblique fractures that Haidukewych et al. discussed. Of the eighty-four nonpathological fractures, sixty-four had the oblique pattern that the authors described, and it should be noted that all of these fractures healed uneventfully without the necessity for either reoperation or bone-grafting. It is often poorly understood that when a screw-plate or nail-plate device is used to fix this subtrochanteric fracture, the stresses are on … |
