Role of CPAP treatment in elderly with moderate obstructive sleep apnea

Popis: Objective: To evaluate the effect of CPAP treatment on clinical, quality of life and neurocognitive spheres in elderly with moderate OSA Methods: Open-label, randomized, multicenter trial in a consecutive clinical cohort of elderly (>70 years old) with moderate OSA(AIH betweeen 15 and 29.9 events.hour-1) to receive CPAP+usual care(n=73) or only usual care(n=72) during 3 months. Diagnosis of OSA was made by respiratory polygraphy or polysomnography and CPAP tritation was assessed by an autoCPAP device. Primary endpoint was changes in the Epworth(ESS) and secondary endpoints were changes in quality of life (QuebecSleepQuestionnaire), sleep-related symptoms, presence of anxiety/depression(HADS questionnaire), blood pressure and neurocognitive tests. Analysis was performed according to the intention-to-treat principle Results: Mean AHI was 21.7(4.8) and 37.8% of the patients showed ESS>10. 72.7% of them had good CPAP-adherence(mean: 5.2(2.5) h/d-1). ESS significantly decreased (-2.6,95%;CI:-3.6 to-1.6, effect size:1)in the group of CPAP. There were significant improvements in the nocturnal symptoms, social and emotional domains of the QSQ. There was a significant correlation between the effect of CPAP and the improvement in EES values and quality of life domains(correlation coefficient between 0.3-0.35,p Conclusions: CPAP treatment resulted in a significant improvement in daytime hypersomnia and some sleep-related symptoms and quality of life domains in elderly with moderate OSA. There was a correlation between number of hours of CPAP use and the magnitude of the improvements
