Usefulness of Brain Perfusion SPET with Tc-99m ECD in High Risk Neonates

Popis: Until now 21 neonates have been studied; 19 of them were preterm (born before 34 gestational weeks) and two asphyxiated new-borns. Imaging studies were carried out between 36 and 44 gestational weeks, at the same time when conventional imagings (ultrasound and magnetic resonance imaging) were performed. Imaging was carried out using a single head or a double head gamma camera with a slant or fan beam collimator. A new brain perfusion agent Tc-99m ethylcysteinate dimer (ECD) was used, a dose of 110 MBq was given. In all 6 neonates with proved cerebral palsy brain perfusion SPET results revealed some pathological changes. All these patients had perfusion abnormalities at least in one cranio-caudal ratio. Slight or substantial sensorimotoric cortical perfusion abnormalities were detected in five of six CP cases. In neonates with normal SPET results no brain damage related to CP could be found. US or MRI revealed pathological findings suggesting CP in four of six proved CP patients. These preliminary results encourage the selective clinical use of Tc-99m ECD brain perfusion SPET studies in high risk neonates.
