LATE-BREAKING ABSTRACT: Exercise-induced bronchial or laryngeal obstruction and vitamin D deficiency in elite athletes

Popis: Exercise- induced dyspnea may be due to bronchospasm (EIB) or laryngospasm (EILO). We found that 25-hydroxy vitamin D (25OHD) deficiency favors laryngospasm during hyperventilation test, detectable by recording maximal mid-inspiratory flow (MIF50). Aim: To investigate if 25-OHD deficiency favors EIB and/or EILO in athletes. Methods: 39 elite athletes, aged 15-19 yrs, 21 (54%) football and 18 (46%) basketball players, underwent AQUA questionnaire, spirometry, treadmill exercise test (ET), and measure of serum 25OHD. FEV1 and MIF50 were recorded before and 1 -5-10-15-20 minutes after the end of ET, as the respective indices of bronchial and laryngeal obstruction: EIB was diagnosed when FEV1 fell by at least 10 %, EILO if MIF 50% fell by 25%. Results: Atopy was found in 28%, rhinitis in 21%, asthma in 13%. All the athletes had 25OHD deficiency: 12 mild (21-29 ng/ml), 21 moderate (10-20 ng/ml), 6 severe (below 10 ng/ml). ET was negative in 14 athletes (36%), showed isolated EILO in 12 (31%), isolated EIB in 4 (10%) and EILO + EIB in 9 (23%). 25OHD was lower in basketball than in soccer players (13.5±1.3 vs 18 ±1.0, p=0.009) and in those with EILO. 25OHD was related to peak heart rate (FC max % predicted) (r= 0.34, p=0.034)and chronotropic reserve (r=0.33,p=0.41). Conclusion: Vitamin D deficiency is common in elite athletes, favors exercise-induced laryngospasm and impairs the chronotropic response to exercise.
