Popis: | An 83-year-old woman developed jaw pain 3 weeks after coronary bypass surgery and mitral valve replacement. Plasma troponin was negative, and the electrocardiogram showed old T-wave inversions. Cardiac CT angiography (Siemens Definition; Siemens, New York, NY), selected because the patient was anticoagulated, showed an occluded saphenous vein graft to the ramus intermedius. In addition, one of the sternal wires was shown to penetrate the anterior wall of the ascending aorta (Fig. 1) with a periaortic hematoma. Anticoagulation was reversed, and the wire was surgically removed. She recovered uneventfully and has had no further symptoms.
